Initialization of a Resource with an Insecure Default vulnerability in Hallo Welt! GmbH BlueSpice (container image bluespice/wiki) allows privilege escalation. The default value of $mwsgTokenAuthenticatorServiceCIDR in the container's LocalSettings.php was intended to restrict internal service authentication to the container network, but evaluated to an empty string when the environment variable WIKI_SERVICE_TOKEN_AUTH_ALLOWED was not set. An empty value disables the IP restriction in the token authenticator component (mwstake/mediawiki-component-token-authenticator). As a result, any user with read access to the wiki (anonymous users on publicly readable wikis) could obtain an application token from the REST API and use it to get a session with sysop privileges as the internal service use

Authlib is a Python library which builds OAuth and OpenID Connect servers. Prior to 1.9.0, the default RFC 7523 JWTBearerGrant.get_audiences() returns an empty list, causing JWTBearerGrant.verify_claims() to require only that an aud claim exists without matching the current authorization server. An attacker holding a legitimate assertion for another service can present it to a server that shares the signing key and uses the default grant, obtaining an access token across a trust boundary. Implementations that override get_audiences() with the server's valid audience identifiers are not affected. This issue is fixed in version 1.9.0.

D365 F&O MCP Server is a Model Context Protocol server for querying Dynamics 365 Finance & Operations X++ code and metadata. Prior to version 1.11.0, the officially documented Azure one-click deployment uses infrastructure/azuredeploy.json and infrastructure/main.bicep to default the apiKey parameter to an empty string and pass it to the App Service API_KEY setting. With API_KEY unset or empty, src/middleware/apiKeyAuth.ts passes requests through without authentication, while src/index.ts binds the HTTP listener to 0.0.0.0 by default. If the deployer leaves the optional key blank and does not add separate network restrictions or upstream authentication, the resulting public App Service allows unauthenticated JSON-RPC tools/call requests to /mcp. A2A Java SDK is the official Java SDK for the Agent2Agent (A2A) Protocol. Prior to 1.3.0.Final, the default InMemoryTaskStore configuration performs no authorization on task reads: the no-argument constructor sets authorizationProvider to null, and InMemoryTaskStore.get(String taskId) returns the stored task without checking the caller or owner. Authorization depends on supplying a TaskAuthorizationProvider and routing requests through AuthorizationRequestHandlerDecorator, whether installed through CDI or explicitly by the application. Default store wiring and manually constructed DefaultRequestHandler.builder().build() handlers, including integrations such as Spring, leave task reads unprotected when no effective authorization-provider and decorator check is installed. Cognition before 0.13.0 contains an unauthenticated remote code execution vulnerability due to defaulting its agent sandbox backend to local execution and shipping no authentication on its REST/SSE API. Attackers can create a session via POST /sessions and post a message via POST /sessions/{id}/messages without credentials, causing the Deep Agents ReAct loop to run attacker-directed shell commands through the local shell backend, resulting in remote code execution as the server process.

Missing Authorization (CWE-862) and Initialization of a Resource with an Insecure Default (CWE-1188) in the MCP proxy (HandleStreamPassThrough in mcp_service.go) and OIDC registration flow (GET /info and POST /register in internal/controlplane/server.go) in Google OSS Agent Mesh (SAM) versions 0.1.0-alpha.1 through 0.1.0-alpha.8 allow a remote attacker to self-enroll into the mesh and invoke arbitrary tools across all connected MCP backend nodes. Because GET /info publicly discloses OIDC configuration, POST /register auto-enrolls any valid OIDC token holder without claim filtering or ban checks, and the default policy binds sam:system:authenticated to wildcard services and targets, an attacker can obtain a mesh credential with full read, write, and execution access on backend services. FileRise is a self-hosted web-based file manager with multi-file upload, editing, and batch operations. From 3.9.0 until 3.22.0, pristine manual FileRise installations without PERSISTENT_TOKENS_KEY or metadata/persistent_tokens.key can activate a published legacy compatibility key. If an attacker separately obtains ciphertext-bearing account or configuration files, the known fallback key can enable decryption of TOTP secrets or credentials. Docker installations already generated a unique persistent key, and existing installations are not automatically rotated because doing so could make encrypted state unreadable. This issue is fixed in version 3.22.0.

Epic Games Unreal Engine versions up to and including 5.8.0 ship with an insecure default configuration for the LaunchURL Blueprint node (UKismetSystemLibrary::LaunchURL). The URL scheme filter (FURLRequestFilter) is empty by default, which permits all URL schemes. On Windows, this results in a call to ShellExecuteW() with the open verb, so a supplied file:/// URI causes the referenced file to be executed. Any Unreal Engine game that loads user-generated content containing Blueprints is vulnerable to command execution via a malicious local file or a remote file on a WebDAV share. This is an instance of CWE-1188 (Initialization of a Resource with an Insecure Default).

MCP Context Forge is an MCP gateway and administrative platform. Prior to 1.0.2, mcpgateway/config.py and .env.example ship a known JWT_SECRET_KEY, while verify_jwt_token() accepts HS256 tokens signed with that key and get_current_user() creates a platform administrator when the token subject matches PLATFORM_ADMIN_EMAIL and REQUIRE_USER_IN_DB is false. A remote unauthenticated attacker can therefore mint accepted administrator tokens without a database account. While the vulnerable signing key remains configured and the platform-admin bootstrap remains enabled, forged access survives password rotation, account deletion, and session revocation, allowing administrative disclosure, modification, and service disruption. This issue is fixed in version 1.0.2.

Due to OVH's default SPF configuration, which commonly includes include:mx.ovh.com, any authenticated user with a valid OVH email account can send messages that appear to originate from any OVH-hosted domains using the default SPF record. SigNoz from v0.8.0 before v0.143.0 defaults the JWT tokenizer signing secret (tokenizer::jwt::secret, set via SIGNOZ_TOKENIZER_JWT_SECRET or the deprecated SIGNOZ_JWT_SECRET) to an empty string, and Config.Validate() does not reject the empty value, so a deployment that does not configure a secret starts up and both signs and verifies session tokens with an empty HMAC key. Because the JWT tokenizer was the default provider, any such deployment is affected. An unauthenticated attacker who knows the ID of an existing user can forge a valid session token for that user — including an administrator — by signing the id, orgId and email claims with an empty key; the organization ID (and whether an email is registered) can be obtained without authentication from /api/v2/sessions/context. The program name and the argument vector that provider executes are taken from the compute provider configuration supplied in the caller's request rather than from operator configuration. An authenticated caller holding only a write role in a single namespace can therefore configure a worker deployment version so that the Worker Service executes a command of the caller's choosing on its own host, under the account the server process runs as.
